JOB SUMMARY:
THIS IS A GRANT-STATUS POSITION.

Under the supervision of a higher classified employee, the Outreach Worker is responsible for making assessments, developing and implementing care plans, and evaluating appropriate records to assess potential need for Outreach Worker services.
EXAMPLES OF DUTIES
Essential Duties:

  • Works closely with other agencies and community organizations to identify potential clients for outreach services.
  • Works with those clients to enroll them in appropriate health coverage and other programs.
  • Documents services given.
  • Works closely with the Department of Human Services to assure client has completed necessary steps for all entitlement programs if appropriate.
  • Facilitates communication between the client and professional/paraprofessional staff, medical care providers and community contacts.
  • Functions as a client advocate.
  • Maintains timely documentation, including care plans and service statistics.
  • Develops and maintains working relationships with area resource agencies.
  • Works with and provides consultation to project team and other Health Division staff.
  • Assists with staff training.
  • Cooperates with other staff in the review and evaluation of services.
  • Performs other related duties as assigned.

PHYSICAL DEMANDS
  • Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a VDT screen and hearing and speech sufficient to communicate in person or over the telephone.

EMPLOYMENT QUALIFICATIONS
Knowledge of:

  • Policies, procedures, rules and regulations related to the WHP and other public health coverage programs.
  • Office administrative practices and procedures, including filing, data processing, computer operations and standard software applications, and standard office equipment.
  • Proper English usage, including spelling, grammar and punctuation.
Skill in:

  • Performing variety of professional level analytical and operations support work without close supervision
  • Using sound, independent judgment within established policy and procedural guidelines.
  • Preparing clear, concise and effective written materials.
  • Maintaining accurate records and files.
  • Establishing and maintaining effective working relationships with those contacted in the course of the work.
EDUCATION
  • Possession of a bachelor's degree in a health, social science, or social services related field, or equivalent experience.
EXPERIENCE
  • One (1) year of experience in providing services to families and/or youth.
NECESSARY SPECIAL REQUIREMENTS
  • Bona Fide Occupational Requirement: Must be fluent in Spanish.
  • Possession of a valid Michigan vehicle operator's license. Must have access to an automobile, van, or truck. Some travel will be required.
