What it means:   Choices & Responsibilities Person Centered Planning (PCP) is a process, not a single event.

What Is Person Centered Planning?

    • Planning is for you by you.

      Person Centered means the planning process relies on your vision of the future.
    • It is a process that unites people who care about you are are committed to helping you plan and build a reliable future.
    • It is a process that allows you to be flexible in making decisions about your life.

What Can It Do For Me?

    • Person centered planning can give you more control and choices in your life.
    • It brings the people you chose, such as your family, friends and clergy into your future planning.
    • It can help you recognize and focus on your strengths while helping you define problems and address any challenges you may have.

What Exactly Is a PLAN?

    • A statement about your dreams and goals (short and long term)
    • A way for you to set priorities and develop strategies to achieve your life goals.
    • In detail the plan identifies your dreams and areas of growth and/or change.
    • Who will help you, and what he/she will do to help you.

All plans have the following qualities:

    • In general they focus on increasing experiences that are valued by you.
    • They highlight your strengths, not your weaknesses.
    • They provide opportunities to gain new skills.
    • They promote a sense of independence in the community.

What Are Some Examples of Desired outcomes?

    • Having friends and/or intimate relationships.
    • Having a place to live that you like.
    • Doing activities that are meaningful to you.
    • Having opportunities to show others what you are good at doing.
    • Dealing with concerns that interfere with reaching your dreams.

What Are Some of the Choices I Have?

    • You can choose people you want to assist you in this process.
    • You can choose to offer direction into meeting your needs and preferences.
    • You can choose to change your plan when indicated.

What Other Supports Does Community Mental Health Offer?

    • Community Living Supports to promote living and participating in the community.
    • Skill building and Community Employment For opportunities to work and experience other meaningful roles in the community.
    • Professional Services:  Nursing, nutrition, occupational therapy, physical therapy, psychology, speech and communication services, consultation, blind services, dental services and specialists in services to individuals with hearing impairments.

    Your services are coordinated with individual primary health care providers. Your health and safety will be a primary concern.


    • Respite Services
    • Both in and out-of-home respite services are available for you and your family.

    What are my responsibilities?


    1.  Attend a planning meeting as small or as large as you would like.
    2.  Give input; identify your dreams, visions, goals, and concerns.
    3.  Assist with the development and implementation of the plan for reaching your dreams.
