Maternal Infant Health Program

If you are a Primary Medical Provider and would like to make a referral to our program call (734) 544-2984 or fax the referral to (734) 544-6705.

Any pregnant woman or baby under 1 year old who is enrolled in Medicaid, Healthy Kids or MOMS may qualify for the MIHP.

This program provides:

  • MIHP provides home-based services where a Public Health Nurse, Social Worker and Nutritionist will visit you in your home during your pregnancy and after the birth of your baby
  • MIHP helps you in accessing transportation for doctor and WIC appointments
  • MIHP helps you apply for social service programs
  • MIHP provides referrals to a doctor or other community agencies to support you and your family
  • MIHP provides access to Childbirth and Parenting Education in the community or your home
  • MIHP provides you with our Nutritionist who is a certified lactation educator to help you prepare for breastfeeding and why it is good for your baby
  • MIHP can make referrals for getting baby items and supplies
  • MIHP provides infant developmental screening and can help access and coordinate Early On Services (see below)
  • MIHP social workers can also provide Infant Massage Instruction for you and your baby

What can this program do for me?

This program can help you get the support you and your new baby need to be and remain healthy. 

How much does it cost?

Our services are absolutely FREE to moms and babies who are on or qualify for Medicaid, Healthy Kids and/or MOMs.
