Community Health CommitteeCHC provides community oversight and networking opportunities for over 200 members of diverse organizations. These include hospitals, government, schools, community-based organizations, neighborhood groups and individual citizens. Newcomers are particularly welcomed to this gathering where we share progress on HIP objectives, opportunities for resource development and collaboration with county partners. Approximately 30-60 members attend each CHC meeting.
Implementation TeamThe Implementation Team is responsible for quality assurance, monitoring health outcomes, interpreting data and information dissemination regarding HIP objectives.
Coordinating CommitteeCoordinating Committee is responsible for tracking progress of HIP objectives, setting agenda for the CHC meetings and activities, and assuring that Implementation Team and Policy Committee workplans serve larger HIP/CHC goals.
