Michigan Tobacco Quit Line: Call 1-800-480-QUIT-NOW (1.800-784-8669)
Who is eligible:

Individuals with no health insurance
-
Individuals with a County Health Plan or Medicare

Individuals completing a brief health assessment and enrolling in quitline
Services:
Individuals can receive up to 2 months of free nicotine replacement pathces, gum or lozenges. Medication will be mailed directly to the caller's home.
Individuals enrolled in telephone coaching will receive printed materials and a QUIT booklet.

Coaching calls are proactive; the counselor will call at a mutually agreed upon day and time.

Coaching calls are designed to motivate quitting and prevent relaspe.

Calls are scheduled around times when relaspe has beeb determined to be most likely.
Individuals with Blue Cross/Blue Shield, ATENA, HAP or others will be encouraged to contact their health plans.
Individuals with Medicaid have some smoking cessation covered as a benefit. Call your healthcare provider for details.
The Michigan Tobacco Quitline is a telephone counseling / coaching program. Intake is from 7a.m. to 1 a.m. EST.
Physicians who refer patients will receive a follow-up report of the patient's progress. Fax referral forms can be printed off at www.Michigan.gov/tobacco under "Information for Health care Professionals".
