Home visiting program for pregnant women and infants with Medicaid. Services include education, service coordination, support and referrals

  • Maternal and infant health assessment and plan of care completed with registered nurse or licensed social worker
  • Coordination of MIHP services with your health care provider and your Medicaid Health Plan
  • Home or office visits provided by a team including a nurse, social worker and dietitian
  • Transportation services arranged, if needed
  • Referrals and support to use other services, as needed  (e.g., mental health services, substance abuse counseling, domestic violence resources, basic needs assistance, childbirth education classes, parenting support classes)
