Maternal Infant Health Program

If you are a Primary Medical Provider and would like to make a referral to our program call (734) 544-2984 or fax the referral to (734) 544-6705.

Any pregnant woman or baby under 1 year old who is enrolled in Medicaid, Healthy Kids or MOMS may qualify for the MIHP.

This program provides:

  • Help with accessing transportation so you can go to medical and WIC appointments
  • Help with applying for social service programs
  • A Public Health Nurse, Social Worker, and Nutritionist to come visit you in your home
  • Referrals to a doctor or community agencies
  • Childbirth and Parenting Education
  • Preparation for breastfeeding and why it is good for your baby
  • Help with getting baby items and supplies, like diapers and formula.
  • Provide infant developmental screening
  • Information to help you find childcare services
  • Assessing infants for EarlyOn Services (see below)

What can this program do for me?

This program can help you get the support you and your new baby need to be and remain healthy. 

How much does it cost?

Our services are absolutely FREE to moms and babies who are on or qualify for Medicaid, Healthy Kids and/or MOMs.
