The Services to Adults with Mental Illness Program provides an
array of services for clients diagnosed with severe and persistent mental
illness. Services are provided based on the level of care and support needed
for the individual as determined in the person centered planning process.
Services are provided by caring and skilled staff. Natural supports and
family are welcomed into the planning process.

Teams
Psychiatrists, nurses, and case managers are skilled in providing
services to meet the unique needs of consumer team participants with severe
and persistent mental illness within their team of service.
- Adult Teams: Services to adults in
an outpatient clinic setting
- Senior Health
Teams: Services to older adults
in an outpatient clinic setting
- Young Adult Teams: Services to young
adults in an outpatient clinic setting
Specialized
Teams & Services
- Assertive Community Treatment (ACT):
A team of psychatrists, nurses, outreach staff and case mangers provides
intensive community support and medication monitoring for adults with
severe and persistent mental illness who require intensive services to
maintain community living.
- Crisis Residential Services
(CRS): A community based
program for clients who are experiencing severe signs and symptoms of
mental illness and require 24 hour support and supervision to resolve the
psychological crisis and return home. Referrals are accepted from PES,
Inpatient liaisons, and case managers. Family and other natural supports
are welcomed into the crisis resolution planning process.
- Dialectal Behavioral Treatment
(DBT): Services designed to
assist clients with serious personality disorders and who meet criteria for
intensive social skill building and development of other coping skills
identified in their person-centered plan.
- Enhanced Community Living Services:
Services designed to address the needs of a growing number of clients
living in community housing developments. A community approach engages the
residents in addressing safety and health issues at home- with the support
of a multidisciplinary outreach team, housing management, various community
resources and health promotion opportunities.
- Family and Consumer Education and Support
(FACES): Service model based on
multi-family psychoeducational groups and case management to assist clients
and their family/friends through education about the symptoms or treatments
for their diagnosis while developing coping skills, social supports, and
vocational opportunities.
- Intensive Case Management
(ICM): Provides outreach
services to individuals who have demonstrated inability to access
outpatient services.
- Jail Diversion
Services: Integrated efforts
between Washtenaw County Sheriff's Department and CSTS to provide
alternatives to incarceration for individuals with mental illness; CSTS
staff also serves inmates who may qualify for mental health
services.
- Meds Plus Teams: Services to adults
in an outpatient setting including solution-focused therapy, medication
management, and case management to clients who have serious and persistent
mental illness.
- Project Outreach
(PORT): A group of community
outreach staff who attempt to engage individuals who are homeless and
resistant to mental health services. Visit PORT's website here.
- Supported Community
Living: Mental health
professionals providing case management, mental health nursing, psychology,
occupational therapy and other ancillary services necessary for client
success in community living for adults with severe and persistent mental
illness.
