Request for Funds - Forms received by the 1st Monday of each month will be processed for payment for disbursement on the 3rd Friday of the month. All others will be held for processing until the following month.

City FY 2007-08 Semi-Annual Performance Report - due
January 11, 2008.
City FY 2007-08 Final Draws (for agencies requesting funds) - due
June 17, 2008.
City FY 2007-08 Year End Performance Report - due July 11,
