Person Centered Planning/Self Determination Workgroup (PCP/SD)

The Person Centered Planning/Self Determination Workgroup (PCP/SD is charged with the development and implementation of a regional Person Centered Planning and Self Determination training.  In addition, this workgroup has developed the regional pool of independent facilitators and monitors their use across the four counties. Each committee has a work plan that is aligned with the Affiliation.
