Job Description


 -Authorizing the signature of the Administrator on the Memo of Understanding
 -From the Center for Healthcare Research and Transformation (CHRT)
 -For the period of 4/1/17 – 7/31/18
 
 -To develop and test multi-payer health care payment and service delivery models for at risk communities
 -To reduce emergency department utilization for individuals with high utilization rates
 
 -The State Innovation Model (SIM) grant will focus on improving care coordination, lowering costs, and improving health outcomes for children and immigrants
 -Case management work will focus on reducing emergency department visits, and may be expanded to focus on chronic health conditions more generally, with a special focus on the Medicaid population
 
 -The work will be completed through a network of safety net organizations, one of which is the Washtenaw Health Plan (WHP) which rests under the Public Health infrastructure
 -The WHP will support case management work and will also be the host fo Community Health workers placed throughout Washtenaw and Livingston counties.

 
 -Place on hold vacant the following 1.0 FTE:
- 1.0 FTE Senior Management Analyst
Group 32, Grade 30
Salary Range: $51,104 - $72,737
 
 -Create the following 1.8 FTE Grant Status positions:
- .60 FTE Community Health Worker
Group 12, Grade 13
Salary Range: $18,262 - $22,353
- .60 FTE Community Health Worker
Group 12, Grade 13
Salary Range: $18,262 - $22,353
- .60 FTE Community Health Worker

 -For a total program of  $ 218,447
- Federal Revenue: $ 210,746
- Local Revenue: $ 7,701
 
 -Upon receipt of the award:
- Authorizing the Administrator to sign the Notice of Grant Award
- Amending the budget
- Authorizing the Administrator to sign delegate contracts
 
 RECOMMENDATION: Approval
 
 
B. Public Safety and Justice
 
 1. Sheriff
 
 a. Annual Community Corrections Comprehensive Plan
 
 -Ratifying the electronic submission of the grant application
 -To the Michigan Department of Corrections
 -For the period of 10/1/17 – 9/30/18
 
 -To continue offering diversion and alternative sentencing options for courts in Washtenaw County
 -To provide pre-trial services, drug testing, electronic monitoring and social education
 
 -In 2016, an estimated 163,357 jail bed days were saved due to Community Corrections Programming
 
 -For a total program of  $ 1,782,798
- State Revenue: $ 650,712
- Program Revenue$ 524,500
- Use of Fund Balance: $110,908
- County General Fund Contribution: $496,678
 
 -Upon receipt of the award:
- Authorizing the Administrator to sign the Notice of Grant Award
- Amending the budget
- Authorizing the Administrator to sign delegate contracts

 -Setting the 2017 Washtenaw County Operating Millage Rate
 
 -The 2017 rate is a reduced from the 2016 rate, due to a Headlee millage reduction factor of 0.9926
