 a. Washtenaw County Public Health Plan of Organization

 -Approving the submission of the Public Health Department Plan of Organization
 -Authorizing the Chair of the Board to sign the Plan of Organization Approval Form
 -To participate in the Michigan Department of Health & Human Services’ (MDHHS) accreditation process
 -Since 2004, the MDCH requires a Local Public Health Plan of Organization be submitted once every three years to include:
- Legal Responsibilities and Authority
- Local Health Department Organization
- Mission, Vision, and Values
- Local Planning and Collaboration Initiatives
- Service Delivery
- Reporting and Evaluation
- Health Officer and Medical Director
 -This Plan of Organization was reviewed and unanimously approved by the Washtenaw County Board of Health at their January 22, 2016 meeting, who are recommending final approval by the Board of Commissioners
 -This is a separate process from the Public Health Accreditation Board national accreditation process undergone in 2015
 b. Prescription for Health Financial Support
    -Ratifying the Memorandum of Understanding
-With the St. Joseph Mercy Health System
-For the period of March 1, 2016 through February 28, 2017
    -To continue the Prescription for Health program in the Chelsea and Ypsilanti communities
-To expand the program to include the Milan community
-To promote the physical health and well-being of low-income and vulnerable populations by increasing fruit and vegetable consumption
 -For a total program cost of $145,000
- Grant Revenue: $145,000
- County Contribution: $0
 -Indirect costs of $23,947 have been included in this budget
 -Upon receipt of award:
- Authorizing the County Administrator to sign the Memorandum of Understanding
- Authorizing the County Administrator to amend the budget
- Authorizing the County Administrator to sign delegate contracts
 c. Appointment of Interim Medical Director
    -Ratifying the appointment of Donald William Lawrenchuk, M.D.
-As Washtenaw County Public Health Interim Medical Director
-Effective January 29, 2016
 -Ratifying the service contract with Livingston County
 -Dr. Lawrenchuk is a licensed physician in Michigan, has an M.D. and an M.P.H. and is Board certified in Preventative Medicine
 -This appointment will remain in effect until a permanent Medical Director is hired, which is anticipated to be within the next six months
 RECOMMENDATION: Approval at Board
