		To evaluate the effectiveness of and disseminate information on the Healthcare Coordination Project To educate medical professionals on strategies to streamline coordination of care and reduce health care costs for persons with developmental disabilities and other cognitive impairments

		For a total program of $84,924: -Federal Revenue: $50,000 -Match: $34,924 (in-kind personnel)

		Total budget includes indirect costs of $0

		Upon receipt of the award: -Authorizing the Administrator to sign the Notice of Grant Award -Amending the budget -Authorizing the Administrator to sign delegate contracts
