2006-2007 Budget Adjustment

• Approving a budget amendment in the amount of $153,901 for CSTS

• Creating a 1.0 FTE Integrated Health Coordination Supervisor position
  -Grade 77, Group 10
  -Salary Range: $48,214 - $64,401

• Creating a 1.0 FTE Nurse Team Leader-Integrated Health grant status position
  -Grade 25, Group 11
  -Salary Range: $43,948 - $57,414

• Creating a 1.0 FTE Medical Case Manager- Integrated Health grant status position
  -Grade 23, Group 11
  -Salary Range: $40,743 - $53,056

• Creating a 0.5 FTE Diabetes Health Educator- Integrated Health grant status position
  -Grade 20/22, Group 11
  -Salary Range: $35,547 - $51,073

• To coordinate, oversee, and expand integrated health services for CSTS consumers including the implementation of a diabetes management program

• Community partners include the Public Health Department, local primary care providers, and the University of Michigan

• Authorizing the Administrator to amend the 2006-07 CSTS-WCHO contract (no additional general fund support is needed)
