4. Washtenaw Community Health Organization (WCHO)

• Authorizing the Administrator to negotiate and sign a one year lease for vacant space previously occupied by Huron Valley Ambulance for review and approval by Corporation Counsel

• To pilot a Recovery Access Center for homeless individuals with substance abuse and co-occurring mental health and substance issues

• Supported by a coalition of agencies including Community Support and Treatment Services, University of Michigan Health System, St. Joseph Mercy Hospital, Center for Behavioral HealthCare, and Huron Valley Ambulance

• Total lease amount is $1 per year

• WCHO will pay for all renovations needed to occupy the space as well as operational costs such as utilities and telephone/computer access

• Lease agreement can be terminated with 90 days notice when the space is needed for 14-A District Court renovations

• Creating one (1) 1.0 FTE WCHO Director – Information Mgt. position:
  -Salary Range: $68,900 - $109,500

• Creating one (1) 1.0 FTE Program Administrator – IT position:
  -Salary Range: $55,900 - $82,800

• To support new contracts and projects related to expansion of the Encompass electronic medical record across the State of Michigan

• Serving individuals whose annual family income is at or below 125% of the poverty level
• CSBG services support client efforts to secure and maintain meaningful employment; attain adequate education; make better use of available income; obtain and maintain adequate housing; and obtain emergency assistance

• Administered in accordance with the State Emergency Funds (SEF) formula, targeting individuals and families with incomes less than 200% of poverty
